What causes elbow pain?
The elbow is a frequently used joint. Pain in this area is often a result of overuse. Repetitive stress injuries like tennis elbow result from injury to surrounding elbow tendons due to repeated movements. Such pain may also be due to sprains, arthritis, or bursitis.
Elbow pain rarely stays put. Because the elbow, wrist, and hand work as one chain, a problem at the elbow often shows up as grip weakness or forearm ache too.
Tennis elbow (lateral epicondylitis)
Tennis elbow is pain on the outside of the elbow, where the tendons of the wrist extensor muscles share a common attachment onto the bony point of the lateral epicondyle. The tendon most often involved is extensor carpi radialis brevis.
Despite the name, most people who get it have never played tennis. It comes from repetitive gripping and wrist extension — trades involving tools, long hours at a keyboard and mouse, lifting with the palm facing down, or any sharp increase in hand-intensive work.
You will usually notice it gripping a kettle or a door handle, shaking hands, or lifting a coffee cup with the palm down. Pressing on the outside of the elbow is characteristically tender.
Golfer’s elbow (medial epicondylitis)
Golfer’s elbow is the same process on the opposite side. The flexor and pronator tendons attach to the medial epicondyle on the inside of the elbow, and the pain sits there instead.
It is provoked by gripping, bending the wrist forward, and rotating the forearm palm-down — so throwing, racquet sports, and repetitive lifting or twisting at work are common sources. Some people also notice tingling into the ring and little finger, because the ulnar nerve passes close by and can be irritated alongside it.
Elbow tendinitis and tendinopathy
The distinction matters more than it sounds. True tendinitis means active inflammation, and it is genuinely short-lived — a matter of days to a couple of weeks. Most elbow pain that has been going on for months is not that. It is tendinopathy: the collagen in the tendon has become disorganised and the normal repair process has stalled, without much inflammation present at all.
This is why anti-inflammatories and rest so often disappoint. They address a process that has largely finished, and complete rest allows the tendon to weaken further.
What a stalled tendon responds to is graded load — controlled, progressive work that stimulates the tissue to remodel — alongside treatment of the joint mechanics and soft tissue feeding into it. Tennis elbow (lateral epicondylitis) is caused by repetitive strain on the extensor tendons that attach to the outer elbow.
