Meniscus injury

Each knee holds two menisci — C-shaped wedges of fibrocartilage sitting between the thigh bone and the shin bone. They deepen the joint surface, spread load across it, and absorb shock. Without them, force concentrates on a much smaller area of cartilage.

Tears come in two broad flavours. Acute tears usually happen when the knee is bent and weight-bearing and the body rotates over a planted foot — the classic sporting mechanism. Degenerative tears happen in older knees where the tissue has stiffened over time, and often follow something entirely ordinary, like standing up out of a deep chair.

Typical symptoms are pain along the joint line, swelling that builds over several hours rather than immediately, a catching or clicking sensation, and sometimes a knee that locks or gives way.

Healing potential depends on where the tear is. The outer third of the meniscus has a blood supply and can genuinely repair; the inner two thirds largely cannot, and treatment there is about managing load and restoring the mechanics around the joint so the remaining tissue is not overwhelmed. A knee that is locked and will not straighten needs an orthopedic assessment, and we will tell you that plainly rather than work around it.

ACL and PCL injury

The cruciate ligaments cross inside the knee and control front-to-back stability. The anterior cruciate (ACL) stops the shin sliding forward on the thigh and resists rotation; the posterior cruciate (PCL) stops it sliding backward.

ACL injuries are usually non-contact. Landing awkwardly, decelerating hard, or pivoting on a planted foot can be enough. Many people describe a pop at the moment of injury, followed by the knee swelling substantially within a couple of hours and a distinct sense that it is not trustworthy underneath them.

PCL injuries are less common and tend to be more direct — a blow to the front of a bent knee, or a fall onto it. The presentation is often quieter, with aching and vague instability rather than a dramatic event.

Our role here is honest and bounded. A complete ACL rupture in an active person is a surgical conversation, and we will send you for that opinion. Where we help is everything around it: screening the injury properly in the first place, addressing the hip and ankle mechanics that contributed, and rehabilitating strength and control — before surgery if you are heading that way, and after it once you are cleared to load the knee again.
